About computer hacking?

If a system is being hacked into via ISP connection just how much can the hackers see? Is it only when I am on-line? If I have an open window can they navigate my history, favorites, can they download my info? Can they see off-line work? i.e. are they hacking in "real-time" exactly when I am on and can they go even deeper into my saved data?
